New Patient Forms & Getting Started
Welcome to NextGen Psyche and Wellness. We make getting started simple. All intake paperwork is completed securely online through our Tebra Patient Portal, so your information goes directly and privately into your chart.
Step 1: Book Your Appointment

Step 2: Complete Your Intake in the Patient Portal
After booking, you will receive an email or text invitation to the Tebra Patient Portal. Please complete everything at least 24 hours before your first visit:
- Patient demographics and contact information
- Insurance information (front and back of your insurance card) and photo ID
- Medical and psychiatric history, current medications, and allergies
- Preferred pharmacy
- Consent for treatment, HIPAA Notice of Privacy Practices, and telehealth consent
- Financial policy and credit card authorization
- Your preferred reminder method (text, email, or phone)
- Symptom questionnaires: PHQ-9 (depression) and GAD-7 (anxiety); ASRS for ADHD evaluations
Step 3: Join Your Visit
- Telehealth (available statewide in California): Join Telehealth Visit
- In person: 5222 Balboa Ave. #43, San Diego, CA 92117
New patient evaluations are 60 minutes; follow-up visits are 30 minutes.
Insurance & Payment
- In-network: Aetna, Cigna, and UnitedHealthcare
- Anthem and other plans: you may be able to see us through Grow Therapy. Book through Grow Therapy
- Self-pay: $350 initial evaluation, $200 follow-up
- Copay assistance may be available for qualifying patients, including veterans. Please ask our team.
Our team verifies your benefits before your visit. Please contact us if your insurance changes.
Important Office Policies
- Card on file: An active credit or debit card is required to confirm appointments.
- Appointment confirmation: Please confirm your appointment at least 24 hours in advance. Unconfirmed appointments may be released.
- Late cancellation / no-show: Cancellations within 24 hours or missed appointments are subject to a $200 fee.
- Minors: We see patients ages 6 and older. A parent or legal guardian must complete the intake and attend the first visit.
